Services/Honoraria for Foreign Visitors with B-1 or WB Visas

1. Visitor needs to have a SSN or Individual Taxpayer ID
2. Fill out Form W-8
3. If a tax exemption is claimed, fill out Form 8233
4. Photocopy passport and visa
5. Fill out Non-Resident Alien Compliance Statement (PDF)
6. Give to Donna Jacobs
